How important do you think continuity of care is from the patient’s perspective?

Read Margaret’s story on page 38 of Integrated Palliative Care and refer back to the video in the previous step where Marlieke discussed patient and family caregivers’ views and experiences about:

  • the care they received
  • to what extent their problems and needs were met
  • their relationships with the professionals providing their care
  • how healthcare professionals work together
