General Practitioner (GP) roles are changing and so is the scope of professional skills needed for clinical practice. Healthcare professionals are more often confronted with complex problems, uncertain diagnoses, and the need to embody a trusted leader.

This four-week course from the WISDOM Project, the Hull York Medical School, and the University of York will help you understand what is expected of you as a GP in the evolving space of healthcare. On completion of the course, you’ll be equipped with the skills and knowledge you need to motivate and sustain change within a community of practice.

Learn to combat insufficient research with problem-solving and knowledge work

In recent years, GPs have frequently encountered problems that aren’t answered by clinical guidelines, existing research evidence, or even medicine.

This course aims to provide you with the ability and confidence to find, create, and use knowledge to make complex decisions about problems faced in everyday practice.

Delve into psychological and community approaches to curb overmedicalisation

Developments in medicine and medical knowledge have allowed healthcare professionals to make leaps and bounds in treating patients and improving public health. However, it has given rise to another phenomenon — overmedicalisation.

You’ll learn to identify and recommend psychological and community-based support as an alternative to medicine in order to provide your patients with treatment that is best suited to their needs.

Today’s generalist practice, caring for individuals though whole-person-centred care, takes more than knowing a little bit about a lot of things. But GPs tell us that their training doesn’t prepare them for the complexities of their job. We learn what we could do to manage disease, but not what we should do to support the individual. WISDOM focuses on the distinct knowledge work skills used by GPs to deliver whole-person, expert generalist medicine.

      This section focuses on what knowledge work is and why highlighting knowledge work and promoting this aspect of primary care is important.

    • Why do we need knowledge work?

      We look at some of the challenges we face when providing medical care and how knowledge work can help us tackle these difficult problems.

    • What kinds of knowledge work can help me now?

      We discuss how different kinds of knowledge work and ways of looking at problems can help us as expert generalists.

    Dealing with too much knowledge

    • Introduction to week 3

      As expert generalists, we need to be able to tailor the care we offer to individuals. Through this module we'll be considering how we can approach medical decision making differently and avoid the harms of too much medicine.

    • Approaching Complexity- Polypharmacy

      We consider adverse outcomes of polypharmacy and when it may be appropriate, barriers to de-prescribing, the 7 steps to appropriate polypharmacy and application of the SAGE framework when managing complex polypharmacy.

    • Approaching Complexity- Medicalisation of Distress

      Considering the overlap between distress and depression and the application of behavioural activation therapy.

    • Closing thoughts

      Here we will reflect on our take home messages from week 3, before testing our knowledge and considering what week 4 has in store for us!

    • Introduction to week 4

      Introducing how GPs can use the knowledge work skills of general practice to become clinician scholars.

    • Becoming a WiseGP

      We have described how you can use your knowledge work skills in everyday clinical practice. Now we will consider how you can apply the same skills to influencing and improving practice in the wider context of general practice.

    • Practical examples of how to apply your knowledge work and expand your portfolio of skills

      In this section, we will use case studies to illustrate how to apply your knowledge work skills to the wider practice environment and think about how you could expand your professional portfolio to shape your future career.

    • Recognising the role of the future WiseGP

      At the heart of our professional practice is the principle that it is not just what you know, but how you use what you know, to improve health and healthcare. This approach gives you the power to re-imagine your role as a GP.

Who is the course for?

This course is designed for primary care clinicians, namely General Practitioners working in the UK and internationally.

Who will you learn with?

First5 General Practitioner, CPCS facilitator, WISDOM project fellow and trainer for the IMPPP trial. Completed a PhD on inflammatory rheumatological conditions and comorbid mood problems.

I am an inner city GP, professor of primary care, and am internationally known for my work on medical generalism. I founded WISE GP and lead the flagship CATALYST programme in Humber Coast & Vale.

First5 GP, GP Programme Director in Dorset, and WISDOM project fellow. Interests in education, workforce research, and championing early career researchers as a member of Wessex REACH NIHR Incubator.

First 5 GP in Scotland. Interested in health inequalities and high quality primary care for all.
