Explore how to make a difference on a local and strategic level

Arguably, long-term conditions are one of the biggest threats to global health. This program will develop your expertise in working with patients living with long-term conditions and their families. You will be supported to consider individual issues and the wider community, in both national and international contexts.

You will explore the importance of person-centred care, self-management and end of life planning. The knowledge gained will enable you to make a major contribution to individuals’ lives, as well as influence the planning and development of services across a range of nursing disciplines.

Who will you learn with?

I had 30 years experience as a specialist nurse, health visitor & community matron before teaching in higher education. Specialist areas are the management of long term conditions & end of life care.

A senior nurse with over 30 years experience in clinical, managerial and educational roles, with a particular interest in specialist palliative care, long term conditions and communication skills.

Learning outcomes

In studying this program you:

  • Critically evaluated the role of the nurse in effectively managing individuals living with a long-term condition.

  • Explored critical factors which influence the quality of care management and delivery in working with individuals with long term conditions.

  • Critically appraised approaches to care which promote health enhancing behaviour in individuals with long-term conditions.
