Explore how nurses facilitate survivorship and end of life conversations

People living with long-term conditions may manage their symptoms and treatment for many years and there may be times when their condition changes or deteriorates.

Facilitating people to live healthily for as long as possible is a key nursing skill, however it is also vital that nurses recognise when end of life is approaching and ensure that the relevant discussions, planning and decision making have been facilitated early in their disease to ensure a smooth transition for the patient and their family.

Advice and guidance will be provided to support the preparation of the final assessment.
