The third experience we want to share is the residency program for the new six-year graduates. In 2015, one of the Taiwan hospital started a general and specialty pharmacy residency. And, the purpose of the general in Specialty Pharmacy residency is to provide leadership in patient care collaborating with other healthcare professionals in the achieving optimal patient outcomes by promoting safe, cost-effective medication therapy. And, the program developed by the hospital initially is a duration for two years, the first year is distributive and second year is clinical.

The reason why it’s a two-year program but the first-year focus on distributive service because the six-year graduate we believe they need to be familiar with the distributive service in the hospital in order they could provide a clinical service better. because they know the whole system how the medication is delivered to the clinics and how the system in pharmacy operate. So, when they go to the second year, the clinical rotation, they could cooperate with the medical team better and they know how to facilitate the physicians and nurses to care of the patient.

So far, the training site not only kept in the original hospital which is Koo Foundation, Sun-Yet Sen Cancer Center, and this residency become a three hospital joint program include the original Koo Foundation Sun-Yet Sen Cancer Center and right now in also include Taipei municipal hospital, Songde branch and the third one Cheng Hsin Hospital. And the residents in this program, they will have four months in each year could rotate through other two hospitals. So, this residency become a cooperative exchange program in Taiwan. And, it become a model for other hospital to build the residency program. This year 2018, TSHP, which is Taiwan Health System Pharmacists, Taiwan Society of Health System Pharmacists is going to start the organization.

They will organize a committee to start the research and the study on how to start a general and Specialty pharmacy residency, and then hopefully the TSHP will promote the pharmacy residency to the whole nation and all the hospital pharmacy will start to run the general pharmacy residency first, and then later, the specialty pharmacy residency could be built based on the general residency. It covers several important section. The first one is core rotations mainly focus on the general medicine. And, the second part is the longitudinal learning experience, mainly focus on the pharmacy service and our resident report and pharmacy advancement requirement. So, these are the core structure of the residency that has been established in Taiwan and this could be revised annually to fulfill the needs for the future. Next, we will share the hospital pharmacist advancement program in Taiwan. The hospital pharmacies advancement program was started in 2011. The purpose is to provide a advancement ladder for hospital pharmacists. The initiator is Taiwan Society of Health System Pharmacist. The program is announced in November 2011 and start to certify P4 in 2013.

In the program the pharmacists’ ladder was divided into P1, P2, P3, P4. And only P4 is required to be certified by the TSHP. So, the four ladders from P1 to P4 mainly focus on essential knowledge and skills of Pharmacy Practice. should be evaluated in recognized first by the department of pharmacy of individual hospital. But P4, which is the highest level should be evaluated in recognized first by the individual hospital and the final recognition is provided by the TSHP. And, there are four domains of each ladder

in the document program: for P1 the seniority which means the length of employment and pharmacy professional competency are the two domains need to be fulfilled for P1 level. For P2 level we add teaching competency into the seniority and professional competency domain. And for P3 and P4 level, in addition to the seniority and pharmacy professional competency and teaching competency, we also add management competency. We also want to share with you the education requirement for dispensing Chinese medicine in Taiwan. The pharmacists with effective licensure and also require additional 16 credit hours traditional Chinese medicine related course, then the pharmacists in Taiwan could handle the training traditional Chinese medicine.

And the 16 credit hours includes the overview with one credit hours, the Pentsaology two credit hours, and formulaology three credit hours, Chinese medicine herbs preparation three credit hours, and Pharmacognosy, total seven credit hours. And this is the basic requirement for Taiwanese pharmacists to handle traditional Chinese medicine.

Other than the postgraduate two-year training program, there are also CPD programs in Taiwan that aims at resident pharmacists. Chiang will be elaborating more about them in this step.

Traditional Chinese medicine related course is required if the pharmacist wishes to dispense traditional medicine. They need to attend a 16 credit hours course including the overview, the pentalogy, formulaology, Chinese medicine herbs preparation and Pharmacognosy.
