Space maintainers

Do space maintainers work? and what can go wrong

Space maintainers can be used in paediatric dentistry to prevent the loss of arch length following premature loss of primary teeth.

Types of Space Maintainers

Fixed Unilateral Space Maintainers: – Band and Loop: Commonly used when a single tooth is lost. It consists of a band attached to an adjacent tooth with a loop extending to maintain space – Crown and Loop: Similar to the band and loop but uses a crown instead of a band

A Distal Shoe can be used if the permanent molar is yet to erupt to guide the molar into the proper position. However these are uncomfortable and not something we would recommend in routine practice. Other less commonly used options include fibre reinforced composite space maintainers

Fixed Bilateral Space Maintainers: – Transpalatal Arch: Used in the upper arch to maintain space across both sides – Nance Appliance: Similar to the transpalatal arch but includes an acrylic button on the palate for additional stability – Lower Lingual Holding Arch: Used in the lower arch to maintain space across both sides

Indications and contra indications for space maintainer use

Indications

Premature loss of a second primary molar Premature loss of a first primary molar in a crowded arch A child with low caries risk

  • Effectiveness: Evidence for space maintainers is at best weak however they are still a very popular treatment. Case selection is crucial as you need to ensure that the benefits out weigh the risks
  • Clinical Situations: They can be useful in cases of premature loss of primary molars, severe crowding, and preventing impaction or ectopic eruption. Clinical guidelines suggest their use should be based on common sense and practical experience

Space maintainers play a vital role in guiding the developing dentition and preventing malocclusion. If you need more detailed information on any specific type, feel free to ask!

Using space maintainers can be very effective, but they can also lead to several complications. Here are some common issues:

Complications

Loose or Dislodged Space Maintainers: – Issue: Space maintainers can become loose or dislodged, compromising their effectiveness – Solution: Regular check-ups are essential to ensure they remain securely in place

Soft Tissue Irritation or Ulceration: – Issue: Poorly adjusted or ill-fitting space maintainers can cause irritation or ulceration of the soft tissues – Solution: Proper adjustment and smooth edges can minimize discomfort

Difficulty with Oral Hygiene: – Issue: Fixed space maintainers can make it challenging to maintain oral hygiene, leading to plaque accumulation – Solution: Educating patients and caregivers on proper cleaning techniques is crucial

Soreness or Discomfort: – Issue: Some children may experience soreness or discomfort, especially initially – Solution: Regular monitoring and adjustments can help alleviate discomfort2.

Breakage: – Issue: Space maintainers can break under pressure, sometimes without the patient realizing it – Solution: Prompt assessment and replacement if necessary

Regular dental visits and good oral hygiene practices can help mitigate some of these issues. If you have any specific concerns, feel free to ask!
