  It is important to look inwards first at post crisis stage to determine how the organization would have handled situations better, thereby learning from it (openness), rather than focusing on external factors and being complacent to the learning needed to be drawn from the crisis experienced.

  I recall a time a new colleague of mine whom I was currently training sent out a sensitive document to the wrong recipients (organizations). The reactive solution I employed to manage the situation, was to personally contact all recipient’s and ensured they disregard and discard the mail.
