If the person has full mental capacity to understand the risks and concerns- you may not make a safeguarding...

  • Hello Susan, it is important to have a discussion with your colleagues, peers, line manager or the safeguarding adults lead- you may also need to discuss this with the VAWG (Violence against women and girls manager who is normally based at the Community Safety Partnerhip in the Local Authority- this is called vital interest-as you said, the person might fear...

  • I agree that it would be great to have some feedback when you have raised a safeguarding concern- our role doe not end there- raising a concern and then forget about it. All organisations need to be able to share lessons learnt, recommendations from these safeguarding adults concerns raised through different methods, such as in peer supervision, team...

  • Hi Christine, when in doubt do not hesitate to share your concerns with a colleague, manager or the safeguarding adults lead :-)

  • Absolutely Susan- it is a whole system approach when it comes to help and safeguarding very vulnerable people- to have opened dialogues with the service user and all parties involved is a very good start. Sharing of information with the service user's consent and involving the service user as in all decision making. Its not something that services can rush...

  • Dear Nadia and Anthony, if you are concern about passing information from hospital to GPs- this needs to be flagged up at a local level- so that both hospitals, GPs and Safeguarding adults in Local Authority are aware of any safeguarding concerns that need to be dealt with.

  • Absolutely Debbie, as you said this is very though provoking scenario which is done deliberately to have meaningful debate and conversation around the subject- as the scenario although fictitious is based on fact and reality.

  • Thank you Rhoda- I think that this needs to be escalated to the Safegaurding Adults Board- via your Safeguarding adults lead, as getting feedback from the safeguarding team is very important for many reasons as primarily in MSP, outcomes of the safeguarding for the individual and how agencies work together to safeguard people,so that they dont fall through...

  • Thank you very much for your kind comments Helen :-)

  • Thank you Jacqui, as per my other comment above. You may also need to contact your Local Authority to ask for feedback adn progress of the safeguarding enquiry- as you or your colleague may be also called to attend the planning meeting with the Local Authority. Do you have a safeguarding committee in your organisations where you can discuss this in a quarterly...

  • Thank you Rhoda for your comment. You may need to contact the Local Authority safeguarding to ask for feedback of the progress of the safeguarding concern you have raised about the service user. You or your colleagues may be asked to attend a planning meeting.

  • @craigm Hello Craig, while I do acknowlede your concern and dilema whether to share or not, especially when a person has the mental capacity to refuse and engage with the safeguarding process. • Hello Paul, thank you very much for this very helpful discussion about S251 of the NHS Act 2006. You are right to assume that it is a statutory power to ensure NHS patient identifiable information to suport NHS activity within health and also where consent cannot be obtained. It allows the disclosure for those wishing to obtain identiable NHS patient...

  • Hi Louise, I have put some guidance for healthcare practitioners to use when having video consultation with service users during covid 19 pandemic.

  • Here is a 30 minutes module for GPs and all health care professionals about information sharing and how it is essential for effective safeguarding. It is a key factor identified in many serious case reviews (SCRs), where poor information sharing has resulted in missed opportunities to take action that keeps children and young people safe. The Data Protection...

  • Dear all, I just wanted to add this guidance for all General Practice staff who are consulting via video with patients at home. Practitioners are expected to have full access to the patients primary care medical record and to be consulting with patients in...

Perhaps your safeguarding leads may be able to offer the half day training via Teams as an innovative way of providing face to face training. Remember that if you are doing other webinars and other...

  • Thank you for raising this Kathryn- good point that we were hoping that more people will pick on that and to create another healthy debate and discussion :-)

  • I just wanted to add that overall, an IMR report should:-
    • set out clearly what issues contributed to the incident, why these problems occurred and what changes are required to prevent the threat, risks and harm by domestic abuse occurring in the future.
•include information about actions, which have already been taken in response to the case and the review...

  • Some additional information about Individual Management Reviews (IMRs) when writing an IMR:-
1 Present the facts and use analysis to explain relevant meaning and lessons that should be drawn out, including that this was good practice that should be emphasised.
2 Be specific on timings, terms such as "regular" and "timely" need to be clarified with a specific...

  • Thank you Sunday. this is a very valid point. As an example, recently I have been involved in a SAR whereby there has been a lack of communication between cross agencies between different London boroughs, in that a person has been moved from borough to borough and from hospital to hospital, and information sharing between boroughs have been lost despite...

  • hello Foteini- thats really good that its provoking some reactive thoughts. I can assure you that it is not a bad script, this video and characters have been done purposely to create meaningful debates and discussions :-)

  • Thank you Penelope for bringing this up. It is absolutely important that GPs to attending safeguarding adults meetings s they also have a lot to contribute. You are right to say that unfortuntely not a lot of GPs have been able to fully participate in those meetings. Having a multi agency representative is vital in decision making collectively to help the...

  • Hi Helen, safeguarding supervision could be done in various ways. it does not have to be formal- discussing complex safeguarding issues with a safeguarding lead to guide and support you either face to face or via telephone conversation is a form of safeguading supervision. but I am glad that you are felt supoprted by your safegarding lead.

  • Hi Thomas, I hope you have learnt that a safeguarding supervisor's role is not to give orders. I hope that you are receiving safeguarding supervision in your work place and that you may also be able to give supervision to others.

  • Hi Faraa, I hope you have been able to make some enquiries with your Local Authority framework for adult safeguarding

  • Hi Alan, at this stage, its only Health professionals who were involved in Sophia's case, but for sure the police will be involved when a SAR meeting is held with the relevant Local Authority

  • Hello all, I am glad to read that you have all found it very useful to have an example of a chronology for a statutory review (DHRs or SARs)

  • Hi Sarah, Marino here. This is just an example of what to include in a chronology. but usually the Safeguarding Adults Board's manager will send their own local authority template for chronology.
