Advanced medication education

We provide not only online counseling, but also telephone and on site counseling. Every counseling session would be recorded by the pharmacist in this system. The system is linking to the hospital medical record system, so the pharmacist can use the barcode on medication bag or patient’s ID number to search and record the medications related to this counseling session. Pharmacists can also use drug name to quickly search for the questions and previous answers recorded by their colleagues. pharmacists should ensure that the information provided to patients, other healthcare professionals and the public is evidence-based, objective, understandable non-promotional, accurate and appropriate. Therefore, the reference is a required field of the record. For inpatient advanced bedside medication education, we focus on the following medication use.
Patient with dual anti platelet therapy and the medications related to comorbidities such as anti-hypertensive drugs, anti-hyperlipidemia drugs after acute myocardial infarction. If the patient needs advanced medication education, doctors, nurses or pharmacists can apply for this service in the system. After receiving this application, the assigned pharmacist will first assess the current prescriptions to prevent any duplication, omission, or unnecessary medications from being prescribed, as well as to serve as an additional review for dosage errors and potential drug interactions. This is the contents of pre-test and post-test of advanced education, including 5 main parts and 1 to 5 points for each part. The first one is does the patient understand drug name, drug appearance and indication?
The second part is does the patient understand how to take the medication? Including the doses, frequency, route and duration. The pharmacist should finally record this bedside education in the system including date of education, duration of education, subject and medications The system produces a simple SOAP note template based on the above recordings allows the pharmacist to complete the SOAP note more efficiently.
