There is also telephone and on-site counseling. Every counseling session is recorded by the pharmacist in this system. Pharmacists provided evidence-based, objective, understandable, non-promotional, accurate and appropriate information. Some patients may require advanced counselling. Physicians can apply for this service via the hospital system. Assigned pharmacist would review each individual application and also conduct pre/pro-tests to ensure desired outcome has been achieved. The system produces a standardized SOAP allowing the pharmacist to record the session more efficiently.
