Pre-consultation survey

Prior to their first physiotherapy consultation, you may wish to ask your patient to complete a pre-consultation survey.

Asking the patient to complete a pre-consultation survey can make your first consultation more efficient and reduce the amount of time you need to take a history. We have provided an example survey provided in the downloads section- it also contains some helpful standardised subjective questions that can be used to reassess pain and function during subsequent consultations. You can refer to the pre-consultation survey, including the patient’s personal goals, as often as needed during each consultation to keep up patient motivation to stick with the exercise and physical activity plans that you will develop together.

When you prescribe home strengthening exercises and develop a personal physical activity plan with the patient, you should consider the patient’s goals and how exercise/physical activity can be best individualised to achieve these goals.
