  • You felt central in developing and agreeing your personalised care and support plan including deciding who was involved in the process
  • You had proactive, personalised conversations which focussed on what mattered to you, paid attention to your needs and wider health and wellbeing
  • You agreed the health and wellbeing outcomes you wanted to achieve, in partnership with the relevant professionals
  • You have a shareable personalised care and support plan which records what matters to you and how your goals will be achieved
  • You have the opportunity to formally and informally review your care plan.
