Before we move on to consider the culture of the NHS, it’s important to remember that even though life has generally improved for people with long-term health conditions and disabilities, we need to ensure that personalised care is implemented well. Otherwise, people will still be at risk of losing their independence.

In this story, Charlotte describes her experience of being put into a care home in her early twenties and how personalised care has transformed her life and the life of her husband, Tom.
