Falls survey

Next week, we are going to explore fear of falling in more detail. In preparation, you may like to fill in this survey.

The link below will take you to a set of questions known as the Falls Efficacy Scale International. This survey is used to measure our fear of falling and to see if it affects our daily life. If you are interested, we would like you to fill out the survey in preparation for next week when we talk more about fear of falling.

The survey will ask you to rate your concern about falling in seven everyday situations (such as getting in and out of a chair, and going up and down stairs etc). Once you have made your choices, you will be given a score which indicates your level of fear of falling. We will ask you to repeat this survey at the end of the course so do please make a note of the score you are given.

This survey is based on the ProFaNE Falls Efficacy Scale International. The data will be collected anonymously and may be used to evaluate the impact of this course.
